  • We offer direct coaching support to current and potential Enhanced Care Management (ECM) providers, often providing shoulder-to-shoulder support with applications, staffing models, program design, policy and procedure development, trainings, needs assessments, strategic planning, workflow redesign, data reporting, analyses, and quality improvement. Our expert consultants worked side-by-side with leadership and frontline staff to develop tailored policies and procedures based on each provider’s unique infrastructure and capacity.   • Our specialized PCMH team will guide you through the entire process to achieve NCQA recognition, meet annual reporting requirements, and complete your accelerated renewal.

    We have assisted twenty-five sites from nine Los Angeles based FQHCs to achieve PCMH recognition. Our 100% recognition success rate is credited to our onsite collaboration with clinic leadership, administration, and care teams. Together, we develop policies and procedures, workflows, and reporting templates to comply with all PCMH criteria, including the most challenging efforts of empanelment, risk stratification, care transitions and management, data visualization, and quality improvement.   • We are committed to providing safety-net providers with resources, tools, and services to train the workforce so that it is better positioned to deliver high quality care and be the “employer of choice”. We offer an array of services, resources, and programs that healthcare providers can use to improve patient experience, health outcomes, and staff satisfaction. Evidence-based training modules include strategies for developing skills to improve customer service, empathy, motivational interviewing, and trauma informed care. We also offer leadership development programs to improve interpersonal communication and team building using the True Colors framework.
