Natalie Martin, MBA, SHRM SCP, TCI Facilitator

With over thirty years of experience, Natalie has helped California’s counties, communities, and health systems in clinical system redesign, practice transformation, and data sharing to deliver patient-focused, accountable, community care. Natalie’s expertise centers on designing and implementing tailored, innovative approaches to evidence-based care, value-based payment, data sharing, community-based care models with multi-sector care collaboration, clinical and social linkages, and technical assistance.
Leading the team at Elevation Health Partners, Natalie works in close partnership with funding organizations, federal, state, and county government entities, and community organizations to identify needs and improve patient experience and health outcomes. Natalie’s expertise includes data sharing privacy, security, technology selection, and implementation. Beyond setting the vision and direction of the company, Natalie serves as the Executive Sponsor of large projects, regularly in the field working shoulder-to-shoulder with healthcare and social services teams.
Natalie is a Society of Human Resources Management Senior Certified Professional (SHRM-SCP) and Certified Facilitator. She advocates for strategic partnership in human resources management by working to align people strategies to organization success and business results. Deena Pourshaban, MPH, PCMH CCE
Chief Operating Officer, West Los Angeles
Deena has over ten years of healthcare and public health experience with extensive work in practice transformation, Social Determinants of Health (SDoH), quality improvement, Patient-Centered Medical Home (PCMH) implementation, HEDIS/P4P and workforce strengthening for collaborative work with clients and care teams to tackle difficult CQMs.
Deena is the Program Director for the Los Angeles Practice Transformation Network (LAPTN), a $16M grant initiative of the CMS Innovation Center to ready practices for value-based payment while demonstrating significant improvements in care for patients with diabetes and depression. She leads a team of 13 quality improvement practice transformation coaches, representing 800 total providers and 23 health centers. As PCMH CCE, Deena leads Elevation Health’s PCMH practice team. To date, her support of 2 clinics at 7 locations achieved 2017 NCQA PCMH recognition. Deena guides the recognition process, developing compliant operational level policies, procedures, and redesigning best practice workflow and staff trainings.
Prior to joining Elevation Health Partners, Deena was the Project Manager of the Policy Analysis Unit at the Los Angeles County Department of Public Health where she collaborated with leaders and executive staff between agencies and held community-based forums on several health economic and impact studies that worked to influence local and state policies.
Aside from supporting Elevation Health’s mission and vision, Deena loves learning about different cultures and customs. She’s had the opportunity to travel to rural areas of Africa and Panama to assist with infectious disease awareness and improve sanitation and hygiene conditions. She enjoys being outdoors, participating in 5Ks, hiking to the Hollywood sign, and climbing the Culver City stairs.
Carol Bernstein, MPH, LCSW
Senior Integrative Health Consultant, San Fernando Valley
Carol has over thirty years of healthcare experience as a LCSW and public health professional consulting, managing, creating, and implementing programs in both clinical and educational healthcare environments.
Carol currently leads three federally qualified health centers (FQHCs) with a combined total of over a 150 providers through clinical and operational transformation towards value-based care as part of the Los Angeles Practice Transformation Network (LAPTN). Much of her work focuses on training staff in empathic inquiry and implicit bias around social determinants of health, developing and enhancing care teams and huddles, defining and creating care navigation, care coordination, behavioral health roles and workflows within the clinic setting, and integrating behavioral health into primary care.
Carol also serves as the team clinical content expert for behavioral health related initiatives, health behavior change, and multi-sector care coordination. She has extensive experience working collaboratively with both leadership and staff teams to facilitate dialogues for change and create new models of care delivery. Her body of work includes creating, implementing and delivering education programs for all levels of professionals and clients focused on communication, motivation, empathy, bias, cultural humility, wellness, mindfulness, resilience, goal setting, weight management, and insomnia.
Stella Gukasyan, Ed.M., TCI Facilitator
Director of Research and Evaluation, Burbank
Stella has over fifteen years of healthcare, education, and public health experience. She has a deep understanding of social and behavioral sciences, HIV/AIDS clinical care and prevention, and developing research projects and data analysis.
Stella is currently leading three federally qualified health centers (FQHCs) with a combined total of over a 150 providers through clinical and operational transformation towards value-based care as part of the Los Angeles Practice Transformation Network (LAPTN). In addition to her experience in the five phases and 27 domains of Practice Transformation, Stella designs and leads highly engaging and interactive True Colors International Personality Assessment workshops, empathy, implicit bias, and cultural humility trainings tailored to clinical and non-clinical primary care and social service settings throughout Los Angeles County. She coaches a multi-site FQHC to achieve PCMH recognition and leads the implementation of a population health management tool.
Prior to joining Elevation Health Partners, Stella was the Principal Investigator and Evaluation Specialist at AIDS Project Los Angeles (APLA) Health for a multi-site Access to Care Initiative in collaboration with AIDS United, Johns Hopkins Bloomberg School of Public Health, and Los Angeles County Department of Public Health.
Stella loves spending time with her family and friends. She has worked in Central America and across different countries in Africa on HIV/AIDS initiatives. She is a published author with over 10 scientific publications, an avid reader, and enjoys exploring the multi-cultural food scene in Los Angeles!
Senior Managing Consultant, Los Angeles
Vincent has over eight years of experience in the healthcare industry, which includes working with solo practitioner offices and Federally Qualified Health Centers (FQHCs). Vincent applies Lean methodologies, Institute for Healthcare Improvement (IHI) principles and tools including Plan-Do-Study-Act (PDSA), and A3 tools to improve various UDS quality measures.
Vincent specializes in health information exchange, data reporting, and system optimization of various electronic health records and population health management systems to further improve clinical data and health outcomes. He also brings extensive expertise in various federal quality initiatives such as Patient Centered Medication Home (PCMH), the Uniform Data System (UDS), Meaningful Use, and Patient-Centered Medical Home.
Prior to joining the team, Vincent was the Health Informatics Analyst at the Costa Mesa-based FQHC, Share Our Selves (SOS), for over three years. At SOS, Vincent managed regulatory and operational reporting requirements including UDS, OSHPD, BPR, PCMH, Komen Grant, HEDIS, and Meaningful Use. He worked on optimizing the clinic’s Electronic Health Record as well as all other ancillary software at the clinic while developing and standardizing clinical workflows to ensure compliance and to improve patient clinical outcomes. Vincent was heavily involved in the empanelment efforts of the clinic as part of its transition to a value-base care model. Rachel Proud, MPH
Senior Managing Consultant, Long Beach
Rachel has over six years of experience working with community health centers, private practices, and hospitals to improve patient experience, clinical quality, and operational efficiency.
Rachel currently leads three federally qualified health centers (FQHCs) with a combined total of over a 100 providers through clinical and operational transformation towards value-based payment while demonstrating significant improvements in care for patients with diabetes and depression as part of the Los Angeles Practice Transformation Network (LAPTN). As the team lead for the Utilization Management Workgroup, Rachel works to select and implement validated interventions to help providers effectively treat patients at high risk for hospitalization and standardize these practices across the network with total network savings of over $130M from reduced ER/hospitalization. Rachel also leads efforts to optimize EHR use and data reporting, engage patients and families, optimize transitions to community care settings after acute hospitalization, increase frequency of medication reconciliation, and improve patient medication education and management in all care settings.
Prior to working at Elevation Health Partners, Rachel worked directly with families and patients as an intake patient advocate at Mission Hospital.
Rachel is a California native and loves going to the beach and Los Angeles Dodger games. Before focusing on a career in public health, Rachel professionally trained in ballet from the age of five, received training in the official school of The Sacramento Ballet and continued her training throughout college
Rula Othman, MS
Clinical Quality Informatics Consultant, Greater Los Angeles Area
Rula brings extensive experience in quality reporting and hospital workflow analysis. Her focus centers on infectious diseases and mental health indicators within displaced populations post-war, as well as the intersection of healthcare and human rights. Additionally, she is certified in Data Analytics through USC’s Viterbi School of Engineering.
Rula serves as a quality improvement coach on the CMS funded Los Angeles Practice Transformation Network (LAPTN) Project, working closely with federally qualified health centers (FQHCs) throughout the greater Los Angeles area to strategize key opportunities for care quality and process improvement.
Prior to joining Elevation Health Partners, Rula consulted at Kaiser Permanente in Population Health Management managing various projects focused on the HEDIS and CMS Medicare 5-star ratings across seven of Kaiser’s West Los Angeles hospitals and medical centers. Her projects included Depression Care, Plant Based Diet, Cardiovascular Health & Rehabilitation, Diabetes Reporting, Hypertension, Rheumatoid Arthritis, Adolescent Metabolic Monitoring, and various other Medication Adherence focused initiatives.
Beyond her role, Rula enjoys expanding her nonprofit work. She currently focuses on education and healthcare access to displaced populations globally through the founding of multiple organizations. Additionally, she is an avid bruncher and loves exploring new spots throughout the greater Los Angeles area.
Kaylie Alexander, MPH
Operations and Quality Improvement Consultant, Greater Los Angeles Area
Kaylie has eleven years of healthcare experience working predominantly in the Los Angeles safety net setting. She has an extensive background in practice transformation, integrative care, project management, healthcare operations, marketing, and development.
Kaylie’s current work at Elevation Health Partners centers on practice transformation. She leads two federally qualified health centers (FQHCs) through clinical and operational transformation towards value-based payment while demonstrating significant improvements in care for patients with diabetes and depression as part of the Los Angeles Practice Transformation Network (LAPTN). As a co-lead for the Health Homes program, Kaylie designs and implements tailored services to CB-CMEs.
Prior to joining Elevation Health Partners, Kaylie worked for over five years at Venice Family Clinic, one of the largest FQHCs in Los Angeles where she helped develop a five-year strategic plan. She was most recently the Program Manager for Dental and Integrative Medicine, responsible for all program operations including measures, outcomes, and budgets. Kaylie increased access and integration for both programs, redesigned workflows using LEAN methodology, and was the lead on numerous quality improvement teams.
In her free time Kaylie is an avid reader who enjoys cooking, hiking and going on adventures in Los Angeles and abroad. She has traveled to five continents and has visited five of the Seven Wonders of the World.
Practice Transformation Consultant, Greater Los Angeles Area
Lauren has an extensive background in population health, network management, and practice coaching, working with IPAs, medical groups, and provider practices. She improves patient satisfaction and the practice transformation initiative from an operational lens. Lauren works toward continuous improvement of the private practices for her projects. She has conducted assessments to address opportunities for practice improvement through side-by-side work with providers and their staff.
With Elevation Health Partners, Lauren focuses on the Encounter Data Project and the Practice Transformation Initiative. She educates offices in quality metrics, ICD-10 coding, timeliness of encounters, and claim filing. Lauren works with practices towards the education of encounter data, the submission process, and the sharing of best practices. Lauren is currently striving toward the improvement of Los Angeles IPA member quality data reporting to positively impact quality ratings associated with HEDIS, STARS, CMC HCC, and RAF scoring.
Previously, at HealthCarePartners a DaVita Medical group, Lauren worked in IPA operations, focusing on private practice management and education. Her practice coaching included quality metric management and improvement, credentialing coordination, compliance, contracting communication, and negotiations, as well as provider education, performance review, and targeted improvement strategies.
Natasha Doshi, MPH
Quality Improvement Consultant, Long Beach
Natasha possesses deep experience and thorough understanding of healthcare policy and current eligibility requirements of Medicare, Medicaid, and Social Security (SSDI and SSI).
Natasha brings her passion for safety net and community health system redesign to Elevation Health Partners. She currently leads two federally qualified health centers (FQHCs) with a combined total of over a 100 providers through clinical and operational transformation toward value-based payment while demonstrating significant improvements in care for patients with diabetes and depression as part of the Los Angeles Practice Transformation Network (LAPTN). Natasha brings expertise in implementing and advancing provider empanelment, care coordination, team based care, and PCMH recognition. As a member of the Los Angeles Workforce Wellness Pilot, Natasha develops interventions to test improvements in order to reduce staff turnover and inform expansion of the pilot.
Natasha’s interest in healthcare consulting began at St. Barnabas Senior Services in downtown Los Angeles where she assisted administrative staff and social workers in coordinating the medical, financial, and housing needs of a low-income older adult population. Natasha currently runs a non-profit organization, Saving Hearts of California Kids, to increase access to automated external defibrillators in schools through community outreach, equipment donation, and training opportunities.
Natasha spends her weekends as a DJ for USC’s student-run radio station, KXSC. She is an avid tennis player and reader of horror novels.
John Carlo Luna, BA
Business Insights Consultant, South Bay (Torrance)
John has over three years of professional experience in the healthcare industry, ranging from extensive non-profit work, research and academia, and expertise in health systems.
John works shoulder-to-shoulder with Federally Qualified Health Centers (FQHCs) around Los Angeles as part of the Los Angeles Practice Transformative Network (LAPTN) initiative. As a LAPTN Transformation Coach, he engages clinics to meet and improve their quality improvement outcomes, transform clinic practice through evidence-based and data-driven decision making, and maximize their electronics health record (EHR) system to its capacity.
John previously worked as a Project Analyst in Kaiser Permanente, providing analytical support to various company initiatives. His proudest work involves developing the analytical framework for reporting the diversity index (DI) of all of Kaiser’s physicians in Southern California in order to develop an innovative curriculum for the future generation of physicians. John has also coordinated a research project on uncovering mental health of the Filipino/Filipino-American women, immigrant, and LGBT+ population through digital media. John was chosen to participate in the CDC Undergraduate Public Health Scholars program (CUPS) and partnered with the University of Michigan School of Public Health where he gained field work with non-profit organizations and community-based research project assessing the sexual healthcare services in Detroit, MI.
Savitri Modi, PharmD
Pharmacist Consultant Liaison, Greater Los Angeles Area
Savitri’s extensive experience includes working on strategies and interventions with the quality improvement practice transformation coaches to embed the Pharmacist into the primary care team.
At Elevation Health Partners, Savitri works as a pharmacist in primary care, retail, and hospital-based pharmacy settings. Her work pulls from these experiences and builds upon Elevation Health Partners experience in providing three years of onsite coaching support to 23 Federally Qualified Health Centers (FQHCs) and more than 70 small and solo practices through the federal Transforming Clinical Practice Initiative (TCPI), an effort designed to help clinicians achieve large-scale health transformation. This work is also informed by three years of participation in the many national learning communities afforded by the program.
Prior to joining the Elevation Health Partners team, Savitri served as a volunteer at a Citrus Valley Hospital with responsibility to monitor anticoagulants, assist with medication adjustments (insulin dose adjustments, TPN dosing for patients, IV to PO conversion, renal dose adjustments), patient medication education for the Parkinson’s Disease support group, collect data for quality improvement projects, and serve as an advisor to IPPE and APPE students.
Savitri enjoys spending time with her family and friends. She is a big foodie and enjoys trying new foods and cooking. She enjoys traveling, reading mysteries, and spoiling her puppy. Additionally, she enjoys volunteering and helping out the community. Through her local temple, she prepares boxed lunches monthly and distributes them to the less fortunate in downtown Los Angeles.
Annie D. Kim, MSW
Business Insights Consultant, North West Los Angeles
Annie has over eight years of non-profit and healthcare experience in spearheading and overseeing projects and programs in community-based healthcare clinics and social services. Her background includes health access strategy, electronic application integration, performance management, quality practice management, revenue cycle, and business development.
Annie collaborates on Health Homes Program (HHP) abstracts to achieve all requirements. She implements programs from scratch, focusing on program design and integration, helping to build capacity, identify workflows, and analyze Tel List to prioritize HHP implementation efforts. Annie also analyzes the program for quality improvements and sustainability. Annie works with six clinics for Health Centered Controlled Networks (HCCN) in their quality improvement efforts around Uniform Data System measurements and with two clinics under Los Angeles Practice Transformation Network (LAPTN) for meeting measurements including sustainability and Value-Based Care Readiness.
Annie recently worked for a Federally Qualified Health Center focusing on vulnerable populations to increase access to services such as primary care, dental, mental health, case management, and pharmacy. She also has collaborated with Health Informatics Analyst and Electronic Health Record Manager to facilitate internal audit and improve multitude of services under Medicaid and Medicaid.
Vanessa Lam, MPH, MPP

Vanessa has a decade of a successful track record in business development, client engagement, health system design, health departments, and clinics. With a background in research, Vanessa brings a strong data perspective to Elevation Health Partners service approach.
Vanessa guides federally qualified health centers (FQHCs) and small and solo physician practices through clinical and operational transformation towards value-based payment while demonstrating significant improvements in meeting the latest standards of care. Her recent projects have focused on health IT data reporting and optimization, gaps in care, diabetes standards of care, integrating the pharmacists with the care team, and improving childhood immunization rates. Vanessa has been staffed on the federal Transforming Clinical Practice Initiative (TCPi) working with the California Quality Collaborative (CQC) and the Community Clinic Association of Los Angeles County (CCALAC) Health Center Control Network (HCCN), funded through Health Resources and Services Administration (HRSA). Vanessa has a strong interest in continuing her work in SDoH and HIV care.
Previously, as Evaluation Coordinator for the UCLA Center for Health Policy Research, Vanessa served as primary liaison for a number of federally qualified health centers, leading their process and outcome evaluations around tobacco cessation. She also supported data analysis projects for center clients and co-authored policy briefs and reports for various projects. Vanessa continued her research and policy role as an Analyst for the UCLA Center for Health Advancement, writing manuscripts for mainstream publication and academic journals, making evidence-based recommendations about how to reduce wasteful spending in the U.S. healthcare system.
In her free time, Vanessa enjoys playing board games and reading at the beach with her dog, Zuko. She is always looking to explore new coffee shops around the Los Angeles area and during her travels.
Pranab Banskota, MBBS, MPH
Senior Public Health and Clinical Quality Consultant, North West Los Angeles
Pranab has over fifteen years of experience in the healthcare industry including work in different countries and across diverse populations. Working as a provider, researcher, advocate, and coach in primary care, as well as behavioral health, Pranab brings a unique perspective to every undertaking.
Since joining Elevation Health Partners in 2017, Pranab helps with Practice Transformation work in Los Angeles Department of Mental Health Clinics. He establishes successful mechanisms of incorporating client feedback in operation of the clinics. Pranab implements tools to boost staff morale at each workplace and demonstrates increased efficiency of the clinics by incorporating quality improvement tools and streamlining processes.
Pranab began his professional career as a physician in Nepal, providing primary care to a vulnerable population in Kathmandu. He transitioned from being a clinician to a public health professional, working in the United States, Nepal, and Qatar, leading several health projects. Pranab enjoys cooking and spending time with his family.
