Los Angeles Practice Transformation Network

EHP served as lead practice transformation coaching firm. 21of 23 health centers completed all transformation phases; clinical quality improvement ranged 5%–33%; 200+ trainings were delivered; and the initiative reported more than $150M in savings associated with reduced ED/hospital utilization.

Michigan Community Health Network – HIPP

Seven FQHCs received direct coaching with network-level support across the CIN. Comparable measures improved 4.6%–17.9% (~11% average), and HIPP centers finished above non-HIPP centers on five of six active measures.

Health Net / Centene QIPM & PQCI

EHP used payer/practice data, barrier analysis and measure-specific coaching across a large Medicaid provider network. PQCI documented 4,047 performance barriers; 57% were workflow-related and 62% of Medi-Cal measure observations improved year over year.

Health Net Equity & Practice Transformation

EHP supported large-scale transformation across data, access, care teams, behavioral/social health, disparity reduction and VBC readiness. By July 2026, coached practices were completing 30% more transformation milestones than practices without regular coaching.
