Elevation Health Partners’ (EHP) Practice Transformation Coaches partnered with seven Michigan Primary Care Association health centers to strengthen HEDIS and CMS quality performance while driving success in Medicaid value-based contracts. The coaches developed practical tools—playbooks, t training decks, and actionable workflows—focused on chronic disease management, care coordination, Transitions of Care, preventive screenings, Well Child Visits, and maternal health metrics.

The result: health centers positioned to maximize shared savings, boost quality incentive earnings, and adopt a scalable model for improvement that can be replicated across the MCHN network and aligned with participating health plans.

EHP led the 2025 bi-annual Workforce Survey across California’s statewide membership, ensuring strong representation across leadership, frontline staff, regions, and demographics.

Leveraging a high-touch outreach strategy—including email, social media, phone, and webinars—EHP surpassed all response goals ahead of schedule. To elevate engagement, EHP introduced a unique incentive program with raffles for consulting hours, process tools, and data assessments.

An AI-driven data analysis was used to generate clear executive insights and tailored summary reports by employee level, making results both actionable and impactful.
