One Practical Structure for Readiness, Technical Assistance, and Implementation
When a statewide or regional program needs a common way to organize readiness, workplans, technical assistance, implementation support, learning, and reporting, EHP can stand up a Rural Health Transformation Hub.
The Hub can support rural primary care organizations and rural hospitals within the same program while allowing assistance to be tailored to each organization's role, priorities, and starting point. It can also incorporate existing partners, vendors, subject-matter experts, and program approval processes.
